Family Physician — Practice Takeover Smithville Family Health Team (FHT) | FHO Model | Office + Inpatient Location: Smithville, Ontario (Niagara Region) Start date: 2026 Practice Type: Full-scope family medicine in a Family Health Organization (FHO) with allied-health supports through the Smithville Family Health Team (FHT). Includes inpatient coverage at West Lincoln Memorial Hospital (WLMH, Hamilton Health Sciences). The Opportunity Smithville FHT is seeking a collegial Family Physician to assume an established community practice from a departing physician. This is a turnkey transition with a stable patient panel and full interdisciplinary supports. The successful candidate will provide comprehensive office-based care and participate in inpatient care/call at WLMH (Grimsby), minutes from the clinic.
