QUEST CHC OFFERS: Competitive compensation (aligned with CHC physician models) PLUS: HOOPP (Pension Plan), Benefits, Professional Development Opportunities Program Area: Primary Care Employment Status: Full Time, Permanent (Part Time may be considered) Hours: Flexible scheduling, including some evenings and on-call participation ABOUT THE ROLE: The Physician provides comprehensive primary care as part of an interdisciplinary team. The role includes clinic-based care, outreach, and participation in team-based planning to support clients with complex health and social needs. There is a strong emphasis on health promotion, disease prevention, and continuity of care. KEY RESPONSIBILITIES: • Providing primary health care to individuals who experience barriers to accessing care • Supporting sexual health and HIV-related care as appropriate or demonstrating a willingness to develop competency in this area • Delivering care across settings including the CHC, community locations, and home visits • Collaborating with an interdisciplinary team to develop and implement care plans • Participating in case conferences, quality improvement, and organizational initiatives • Supporting continuity of care through coordination with hospital and community partners QUALIFICATIONS: Licensed to practice in Ontario with the College of Physicians and Surgeons of Ontario • Certification with the College of Family Physicians of Canada (CCFP) • Membership in good standing with CMPA EXPERIENCE & SKILLS: • Experience working with priority populations, including those experiencing mental health and substance use challenges, housing instability, and barriers to care • Experience or interest in sexual health and HIV-related care • Strong commitment to harm reduction, trauma-informed, and client-centred care • Ability to work collaboratively in an interdisciplinary team environment • Excellent communication, clinical judgment, and problem-solving skills If you are looking to practice in a supportive, team-based environment where you can build meaningful relationships and contribute to community impact, we would love to hear from you.
